What is a Dual Special Needs Plan?

A Medicare Advantage plan that combines Medicare and Medi-Cal into one plan. Plans like these are often called Medi-Medi plans.

What are the benefits of a D-SNP plan?

With Original Medicare or a standard Medicare Advantage plan, you may have premiums (monthly fee) or copays for doctor visits (upfront costs at the clinic). With a Blue Shield TotalDual Plan (HMO D-SNP), you pay $0 premiums and $0 copays for covered services and dental coverage at no cost. Plus, you get allowances/benefit coverage for:

  • Food and Produce

  • Over-the-counter items

  • Hearing aids

  • Vision care, including eyeglass frames, lenses, or contact lenses

Am I eligible for D-SNP?

To enroll in a D-SNP, you need to qualify for both Medicare and Medi-Cal. At Blue Shield, we offer Blue Shield Total Dual Plan (HMO D-SNP) to Los Angeles and San Diego County residents.

Every Blue Shield TotalDual Plan (HMO D-SNP) member gets a dedicated care coordinator at no extra cost. Their services are available in multiple languages.

Care coordinators can answer questions about:
 

  • Your health care

  • Getting behavioral health (mental health and substance use disorders)  services

  • Dental benefits

  • Transportation to medical appointments

  • Long-term Services and Supports (LTSS), including Community-Based Adult Services (CBAS) and Nursing Facilities (NF)
